Medicare Supplement and Plus Endorsement Paper Applications Updated

We have updated the Medicare Supplement and Plus Endorsement paper applications with a few changes.
Billing options on paper applications have changed
Just as it is shown in SalesConnect, we only offer monthly bank draft or monthly billing for payments. The Medicare Supplement and Plus Endorsement paper applications now mirror that billing option.
If a member wants to make a one-time credit card payment or setup recurring credit card payments, they will need to call customer service at
Combining BlueCare® and Blue Select®
Another change we made was to combine BlueCare and Blue Select plans into the same application. The new applications, one with health questions and one without health questions, are available for download on Storefront.
We want to encourage you to continue to use SalesConnect to process your Medicare Supplement applications.
